If you are having an outpatient radiology exam at a Washington Imaging Services imaging center in Bellevue or Issaquah, you can review and complete various forms prior to your scheduled appointment. Follow the links below to the WIS Web site where the forms are available in PDF format. They can be printed out, completed and then brought with you on the day of your exam or faxed over in advance.

Patient Registration Form (Required of all patients.)

Patient History/Health Questionnaire (Required of all patients.)

CACS Patient History Form (Required of all patients having a CACS exam.)

HIPAA or Privacy Notice (Required of all patients. If you’ve completed this form at a prior visit to our office, you do not need to fill it out again.)
